Information for your first appointment with us:
Our office has become paperless. When you make your appointment with us, you will receive a link through your email registering you for our patient portal. This link to register on the portal will expire after 72 hours. Please login within 72 hours of booking your appointment to complete the necessary patient information needed prior to your visit. If your link expires please call our office to receive a new registration email link at 817-380-5911.
If you cannot complete the patient information prior to your visit, we ask that you arrive 20 minutes early to your appointment to complete your patient information on our patient kiosk.
